I made a filament guide to direct the filament from the spool towards the extruder when used with a top mounted spool holder.
The opening for the filament is on the back of the thing with a small opening that is not big enough for filament on it's own to pass through, but when pulled apart slightly it is big enough.
I have also used a PTFE tube of around 30cm long to guide the filament smoothly towards the extruder, this to prevent it from unrolling the spool on it's own.
It goes really well with my Spool Holder, they fit snugly together on the top part between the LCD and the Z Top mount;
