This is a mount for a Watercool Heatkiller D5 reservoir that has holes aligned for a 120mm fan slot. This is intended to be mounted horizontally in your computer, so the reservoir is standing upright.
I already installed it in my case before I could take a good picture of it all in isolation. Its a pretty quick print though, give it a try. This is V2, where I added studs to hide screw threads for attaching this to the bottom of your case where a 120mm fan would sit. I suppose you could attach this on top of a fan with some modification (remove the screw thread caps and widen the holes), but that's not how I intended it.
