This mounts a 40mm fan onto an SDR such as (in my case) a Nooelec NESDR which despite being metal-cased gets warm in sustained use.
The bottom is flat for the fan, with holes for (countersunk) screws. The top has steps to suit a cross-section of between 12.5 and 17.5mm with a hole for a cable tie to hold things together.
Overall size is suitable for e.g. a Dell desktop computer which has fairly closely-spaced USB slots on the front panel.
The original FreeCAD file is provided, laid out such that any user should find it easy to adjust the size of the fan mounting holes or SDR steps.
