This shield covers both front-facing cameras and all three rear cameras (as shown at https://www.att.com/devicehowto/index.html#!/devicediagram?make=LG&model=LGLMV405UA). I'm not sure It is specifically adapted to work while the LG V40 has the Otter Box on. However, you could change it as necessary using the blend file (You can make the outer part about 2mm thicker in each dimension (resulting in 1mm thickness on each side) in comparison to the phone object which becomes the hole.
