You will need all parts printed as it has been designed to be assembled as per the images. You may be able to get away with a different layer fan assembly, though. The X Axis bracket endstop tab has been removed to allow for extra movement to allow the hot end to be centralised. The mount will only currently work with the SN-04 auto levelling sensor due to the mount bringing the X Axis very low to the bed (~5-7mm) as the nozzle will be the only thing under the X axis.
This mount will require some heavy firmware alterations to get things printing correctly.
