This is the top and bottom plates for a DJ TechTools Midi Fighter Classic (Arcade Button DJ/MIDI/Music Controller).
It's great as a custom or replacement top plate for a Midi Fighter classic, since they are no longer for sale. It also works great as a top and bottom plate for any DIY MIDI or arcade button controller using 24mm Sanwa arcade buttons.
Lastly, they work great as stencils if you'd like to use other materials such as wood or metal.
**Update: I've created and added the side enclosure to make the full controller. You can user 24mm arcade buttons or 3D print these: http://www.thingiverse.com/thing:1400499 (Just scale them up 190%) and mount a small/cheap tactile button (https://www.amazon.com/gp/product/B008420WOA/ref=as_li_ss_tl?ie=UTF8&psc=1&linkCode=sl1&tag=kmpro03-20&linkId=9e9b3ddd48c8b34ca37e9edd5f0f3128) inside them and you have a fully functioning/fully printed midi controller.
