After many, many revisions I've finally got a version that I'm happy with. This will allow you to use the newer LXT (Li-ion) batteries with an older PA18 (Ni-Cd) Makita drill.
I used a cheap Chinese LXT clone battery to test and design this (will source some legit Makita batteries at a later date).
You will need a small 0.28mm voltage tester and a small SPDT push switch if you want the voltage display.
Some M3 (2x16mm, 2x20mm) Hex head screws and M3 nuts.
You also will need the official Makita LXT terminal part number #643852-2 (which is super cheap on ebay).
The clips could be better, they (if anything) work too well; had trouble removing the adapter.
The PA-18 Insert "The Stick" was designed to use some 8mm x 0.5mm x 100mm nickel spot welding strips (cut in half) - from here I've soldered the 14awg wires to some connector spades. You will need to use the central channel to cross over the cables as + and neg are reversed on the PA18 compared to the LXT
