This is a modified extruder body as well as extruder idler to allow your MK2s or MK2s clone to use a more standardized MK8 hobbed bolt. It shifts the motor mounting points over by 1mm to make room for proper alignment of the filament into the PTFE tube. (MK8 hobbed bolt has an effective diameter of 7.5mm, whereas the prusa one was only 6mm)
In Tom's "MK2 clone" build series on youtube, he drilled his holes out slightly to allow for his motor to be better aligned, but it still wasn't perfect and his extruder motor was not properly aligned. This solves that problem.
