This is a cap made for a 3/8" GRK brand RSS (Rugged Structural Screw) with a T-40 torx drive.
There is a small round stud in the center of the cap that wedges in between the points on the torx head to hold it in place. Line it up square and tap it in with a soft mallet.
I made this because I am using them to hold an offset umbrella down to my deck and wanted something to cover up the screws to keep water off them. I don't know how long they will hold up in UV from the sun or over the winter but it's easy enough to print new ones if needed.
