This is a basic dual pantograph jack with around 40mm Z travel. It uses 1/4" threaded rod for both the main screw, which drives against two captured hex nuts, and for the through-hole connections between scissors. It also uses 8-32 bolts with lock washers for the scissor pivots, and M3 bolts and hex nuts for the clamps. See pictures for more details.
