I made this because I broke my ankle in a car accident and trying to carry a coffee cup and walk on the crutches at the same time is a PITA.
You will need to print one of the actual holder, and 2 of the hinge/bracket pieces.
To put it together, you pop the end of the crutch off, take the grip off, and the two pieces should slide down either side, and then you spread them apart to insert the cup holder. I recommend sliding it down as far as possible, as it will create a slight interference fit. There is also a hole on the side of the brackets if you want to pre-drill and add a screw for a bit of added security. Be mindful, that the grip/handle will spread the bars apart. I went through several revisions and prints before I realized this.
I have included the sketchup files, so feel free to make changes and remix it.
Parts should already be in the correct orientation for printing.
